AMENDMENT NO. 1
TO
PROFESSIONAL SERVICES AGREEMENT 2425-157-A1
BETWEEN
CENTER FOR PUBLIC SAFETY MANAGEMENT, LLC and THE CITY OF
UKIAH
This Amendment No. 1, entered on June 24, 2026, revises the Agreement for
Professional services dated December 19, 2024, between the City of Ukiah and Center
for Public Safety Management, LLC., for professional consulting services relating to
UVFA Strategic Planning Services.
This Amendment No. 1 adds an additional Scope of Work and an additional $3,350, as
described in Exhibit 1. This revises the total, not-to-exceed amount to $65,500.
Except as expressly amended by this Amendment, all other terms remain unchanged and
in full force and effect.

The Center for Public Safety Management, LLC can provide a formal presentation of the
Strategic Plan document prepared for the Ukiah Valley Fire Authority for the following fees:
✓ One presentation.
✓ Presentation fee - One associate virtual, one associate in person: $3,000.
✓ Travel expenses: Estimated $350.
✓ Total fees not to exceed: $3,350.
✓ Fee proposal submitted: June 23, 2026.
