AMENDMENT NO. 2
TO
PROFESSIONAL SERVICES AGREEMENT
BETWEEN
REN ALEXANDER DESIGN and THE CITY OF UKIAH
This Amendment No. 2, entered on July 22, 2024, revises the Agreement for Professional
services dated March 31, 2022, between the City of Ukiah and Ren Alexander Design for
professional consulting services relating to providing architectural services and plans for
the interior of the Electric Service Center at Hastings.
This Amendment No. 2 adds an additional $15,600, as described in Exhibit 1. This
revises the total, not-to-exceed amount to $98,100.
Except as expressly amended by this Amendment, all other terms remain unchanged and
in full force and effect.

REN / ALEXANDER Ukiah Electric Service Center T.I: Change Order Request
CHANGE ORDER REQUEST
Hello Cindy,
We are writing to formally request a Change Order to address Construction Administration services
required for the ongoing tenant improvement project at 1350 Hastings Ave.
As we progress through construction we project to bill to the total original contract sum within the
next four to six weeks.
This is in part due to a number of factors, including previously billed Additional Services during the
Construction Documents phase and the Bidding & Negotiation phase. As requested, additional hours
spent during these phases were billed as worked toward the base contract. The combination of
existing building conditions and the fire and water damage are necessitating more time spent
providing responses to RFI’s, supplemental information/ sketches and coordination during
Construction.
To maintain the quality and integrity of the project while adhering to the proposed schedule, we
respectfully request a Change Order to cover additional Construction Administration costs. This
Change Order will ensure that the Ren / Alexander can continue to provide the required level of
oversight and support for the team to successfully complete the project.
