2. That the nature of the work: Installation of underground conduits for 12kV primary conductor
in three areas on the western side of Ukiah. Specification No. 23-08.
3. That on the 6t6 of June 2024,the Contract work for this project was actually completed.

5. That the property location of work performed herein referred to is situated in the County of
Mendocino, State of California, and is described as follows: 100 Block of West Standley St..
