CITY OF UHIAH
AMENDMENT NO. 2
TO
PROFESSIONAL SERVICES AGREEMENT
BETWEEN
PSOMAS and THE CITY OF UHIAH
This Amendment No. 2, entered on August 18, 2021, revises the Agreement
for Professional services dated September 9, 2019 between the City of Ukiah and
Psomas for professional consulting related to land surveyor services.
This Amendment No. 2, as per Attachment 1, requests revisions to the previously
provided Annexation Exhibit Map and Legal Description to remove the APNs noted as
Area 4 and prepare an updated Exhibit Map and Legal Description without the prior Area
4 APNs.
This Amendment No. 2 adds $2,600 to the agreement, for a total revised contract amount
of$48,135.
Except as expressly amended by this Agreement, all other terms and conditions of the
agreement remain unchanged and in full force and effect.

Amendment No. 2 — Revisions to Annexation Map and Legal
Dear Jesse:
Please accept this letter as Amendment No. 2 to your Professional Services Agreement for the above
stated project. The changes to your scope of work, schedule and/or compensation are noted below.
This authorization shall be in accordance with and pursuant to the terms of the fully executed
Professional Consulting Agreements dated September 9, 2019 between Psomas and City of Ukiah.
Scope and Schedule Changes:
Psomas will perform the following Scope of Work:
Revise the Annexation Exhibit Map and Legal Description previously prepared by Psomas to remove
the APN's previously noted as Area 4 and prepare an updated Exhibit Map and Legal Description
without the prior Area 4 APN's as noted on the attached Exhibit "A".
The updated Legal Description will reflect the revised metes and bounds courses and course index
numbers. The metes and bounds course index numbers will be updated on the course index numbers
and in the line tables on the Exhibit Map. Individual areas will be noted and shown for the unaffected
Areas and the updated overall area will be noted in the Legal Description and on Exhibit Map,
Deliverables:
Provide signed and stamped copies, in digital format of the Exhibit Map (.PDF and .DWG files), and
.PDF of the updated Legal Description,
Schedule:
Psomas will begin the above scope of work within 3 days of receipt of an executed PO.
Preliminary deliverables will be submitted with 5 working days of commencing the above Work.
