AMENDMENT NO. 1
TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES
DATED MAY 10, 2018
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Consulting Services dated
May 10, 2018 shall remain in force and effect except as modified by this first amendment.
AUTHORIZATION
Upon execution of this amendment, service provider is authorized to provide the service
described under the Scope of Additional Work identified in this Amendment No. 1.
SCOPE OF ADDITIONAL WORK
Work authorized by this amendment shall include that work described in the
September 3, 2019 letter proposal from GHD. A copy of said letter is included in this first
amendment.
COMPENSATION
Compensation for the performance of the additional work shall be made on a time and
expense basis at Consultant's customary fees. Expenses shall not exceed a maximum
total cost of $13,930.

GHD is pleased to provide this proposal for an Additional Services Agreement (ASA# 1), for services
related to optimizing the configuration and circulation within the Ukiah High School student drop-off/pick-
up parking lot. The ASA is intended to modify nothing in the original agreement dated May 10, 2018 and
referenced above, except the description of Services to be Performed (A), and Compensation (B), as
follows:

Description
Conduct site visit to collect measurements for the current student Ukiah High School drop-
off/pick-up parking lot, including all entry driveways, sidewalks, parking stalls, median/tree
wells, and driving lanes. Observe use during one afternoon departure period.
Prepare two preliminary circulation alternative Improvement Maps for the student drop-
off/pick-up parking lot. The alternatives will likely consist of a lower-cost alternative (focused
on changing signage, striping, and instructions to students/parents), and a higher -cost
alternative requiring changes to hardscape within the parking lot. Deliverables will be
provided in .PDF.
Develop planning -level cost estimates for preferred alternative, by improvement type, to
calculate preliminary project cost estimates for proposed circulation changes. Deliverables to
be excel list of improvements, with associated costs, in addition to cost estimate backups.
Prepare preliminary designs for the preferred alternative selected by the client, with
modifications to the draft alternative as requested by client. Prepare supporting
documentation outlining circulation procedures for caregivers, students, school busses, staff,
and visitors. Documentation might include narrative for Student/Parent Handbook, flyer for
Back to School distribution, and a simple annotated map. Deliverables will be provided in
.DOCX and .PDF as appropriate.

B. Compensation for Services:
Total compensation for services will be $13,930 on a time and expense basis, broken down by staff in
estimated hourly increments by task as follows:
