HomeMy WebLinkAboutLACO Associates 2019-03-08; Amendment 1 2019-06-11timum,. _ ________ CITY OF UKIAH AMENDMENT NO. 1 TO PROFESSIONAL SERVICES AGREEMENT BETWEEN LACO ASSOCIATES and THE CITY OF UKIAH This Amendment No. 1, entered on June 11th, 2019, revises the Agreement for Professional services dated March 8th, 2019 between the City of Ukiah and LACO Associates for professional consulting services relating to consulting services related to the preparation of two Hazard Mitigation Grant Program Notices of Interest for DR -4407. 1. Amend scope of work to include two sub applications for submittal to the HMGP program as per Exhibit A. 2. Contract Term: LACO will prepare and deliver completed sub application materials to City for review by June 26, 2019 and submit the sub applications by the July 5, 2019 deadline. 3. Add an additional not -to -exceed amount of $15,500, for a total revised not -to - exceed contract amount of $19,500. Except as expressly amended by this Amendment, all other terms remain unchanged and in full force and effect. PROJECT UNDERSTANDING LACO previously supported City in developing two Notices of Interest for the program: 1) advance tree trimming around distribution lines and 2) switching equipment, insulated conductors, and undergrounding overhead conductors. The Notices of Interest were approved by the California Governor's Office of Emergency Services (CALOES), the agency which provides initial approval of projects, and City has requested further support to develop the complete subapplications for the projects. SCOPE OF SERVICES LACO, in consultation with City, will prepare two subapplications for submittal to the HMGP program. LACO will develop application narratives, budgets, scopes of work, and schedule, along with other supporting documents listed below. Task 1000 2000 Task Description Estimated Fee Wind Hazard Subapplication Fire Hazard Subapplication Total Estimated Fees $10,500 $5,000 $15,500 Task 1000: Wind Hazard Subapplication LACO will develop an HMGP subapplication which will include the following sections: • Project subapplication form and all narrative sections • Project Scope of Work • Project Designs • Project Mapping • Project Schedule • Project Cost Estimate and Narrative • Benefit -Cost Analysis and associated narratives • Project Environmental Checklist and narrative LACO will compile the subapplication materials into a compliant submittal package and, at City's request, submit the application to CALOES. Task 2000: Fire Hazard Subapplication LACO will develop an HMGP subapplication which will include the following sections: • Project subapplication form and all narrative sections • Project Scope of Work • Project Designs Page 1 of 2 • Project Mapping • Project Schedule • Project Cost Estimate and Narrative • Benefit -Cost Analysis and associated narratives • Project Environmental Checklist and narrative LACO will compile the subapplication materials into a compliant submittal package and, at City's request, submit the application to CALOES. DELIVERABLE Electronic copies of all subapplication materials will be delivered to City on a compact disc or flash drive. SCHEDULE City will provide requested documents, to the extent possible, by May 31, 2019. LACO will prepare and deliver completed subapplication materials to City for review by June 26, 2019, and submit the subapplication by the July 5, 2019 submittal deadline. FEE LACO will invoice City based on time and materials. The estimated fee is $15,500. SPECIAL CONDITIONS, ASSUMPTIONS, AND SERVICES NOT PROVIDED • LACO does not make any guarantee of the viability of the project, the date upon which approvals will be received, or the successful completion of the project. LACO does not possess any control over the discretionary approval process employed by any involved government agency, or the timely processing of application submittals. • City will pay all public agency fees associated with this project. • Task budgets are estimates of required effort; to meet project objectives, LACO may adjust the budget allocations for tasks, but may not invoice in excess of the total estimated cost without written approval from City. • Tasks not specifically identified in the Scope of Services above are not included. • Application compliance is first assessed via a review of the project benefit -cost analysis. If the project does not generate more benefits in dollars than the lifetime cost of the project, CALOES will not review the subapplication. All project components proposed in this scope of services will be input into the Benefit Cost Analysis software as the first step in application development, and any project components which do not yield an adequate benefit -cost ratio will need to be excluded from the subapplication. • City will provide requested project information, as available, including: o Documentation of estimated project costs o Mapping or coordinates identifying project locations o Scope of work information, including any existing design drawings or standard designs o Any vulnerability studies or other background information which contributed to project selection o Photos of project locations o Implementation scheduling information o Match and maintenance commitment letters o Maintenance cost documentation o Supporting documentation regarding historical outages and other impacts to infrastructure being protected, to the extent available o Agent Authorization Form o Subrecipient Grants Management Assessment Form P:\6800\6883 City of Ukiah\6883.35 Electrical Utilities Hardening HMGP\02 Service Agreements\Draft & Unsigned Agreements\6883.35 Amendment No. 1 Exhibit A 20190515.docx
