AMENDMENT NO. 1
TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES
DATED MARCH 20, 2017
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Consulting Services dated
March 20, 2017 shall remain in force and effect except as modified by this first
amendment.
AUTHORIZATION
Upon execution of this amendment, service provider is authorized to provide the service
described under the Scope of Additional Work identified in this Amendment No. 1.
SCOPE OF ADDITIONAL WORK
Work authorized by this amendment shall include extending such work described in the
February 28, 2017 letter proposal from SHN Consulting Engineers & Geologists, Inc. A
copy of said letter is included in this first amendment.
COMPENSATION
Compensation for the performance of the additional work shall be made on a time and
expense basis at Consultant's customary fees. Expenses shall not exceed a maximum
additional total cost of $5,000 for a total contract cost of $21,520

Thank you far this opportunity for SHN Consulting Engineers & Geolugists, Inc (Sl IN) to offer the
following proposal to the City of Ukiah (Cit>;) to provide Construction Materials Testing for the LVater
anti Seiler Replacement Project (Project). Follimtirig is a brief discussion of SHN s construction
materials testing services yualific-itioits, our project understanding including scope of scars i.. es, and our
proposed fee estimate.
Firm Qualifications
Sinn's construction materials testing staff is coniprised of citil enginctirs certified construction
inspectors, and niaterial testers. Our field and laWrator) testing technicians are certified by Cahuui%
American Concrete Institute (AC[), National Institute for Certification in Lngineefing'iechnoiogies
(NICEI'), Internationril Cnde Council KC) and our laboratory is accredited by Caltrans and the
AASI ITO Accreditation Program (AAP) Our laboratory and field technicians use state of the art,
calibrated testing eyuipnient to protide accurate and timely construction materials compliance analysis

Project Understanding and Scope of Work
Our project understanding anti scope of work is based upon a review of the Project plans and
specifications and the specific items of work identified in the Reyutrst for Proposal (RF1') received on
February 2 t, 2017. Following is a discussion of SI IN's approach to providing the specific construction
materials testing cervices oullimd in the RFP.

Scheduling and Repotting of Test Results
'I'he request for proposals (RF11) indicates a construction start date of February 27, 2017, however
compaction lesting of trench backfill is not anticipated until mid-March 2017. SHN will be in direct
conLact with the City Engineer or his designee for scheduling of any field sampling and/or testing
at.tivities, wide a minimum notice of twenty four hours. Test results of compaction tests will be provided
to our client representative in the field immediately upun completion of testing, followed by a written
test report within a forty eight hour period. Results of all laboratory testing will be reported in writing
within a forty eight hour period from the time the testing is completed.
Compaction Testing
based un the anticipated amount of individmil compaction testa requesled in the RFP (88 on
sewer/water trench backfill & I I on lateral trench backfill), and the cumulative total length and depth of
utility trench, and the amount of work days anticipated, we estimate an average of three cnnttpacbon
tests per day. with a testing frequency of one test per two hundred linear feet of pipe installed, at depth
intervals of approximately taelu a inches, for a total of '43 {lays. We propose to provide 2 hours of ort -Site
compaction testing services for each site visit. Lateral treitrhes can he tested for relative compaction on a
random basis at the discrction of the project inspector. Our tasting technician will also provido subgrade
anti aggregate base mmpactiuit testing for the roadway improtements upon the completion of thte sewer
anti water mains installation on each of the three roads. All soils compaction testing will be performed
in accordance with Caltrans method Cr 231 (nuclear method). Subsequent asphalt paving will be tested
ill accordance with Caltrans hst method Cf' 375 (nuclear method) We propo.%: to perform th; asphalt

of asplialt is at least 300 tons or greater.
Materials Testing
The testing technician will collect construction material, samples as required ill the Project apttificatnorts

testing and lead time for laboratory testing and reporting of test results are typically isithin 48 hours of
sampling in the field. A minimum of one full five gallon bucket of material, sampled in accordance with
Caltrans sampling method Cl' 125, is required to ensure tine minimum amount of materials to perform
laboratory quality testing. Sampling events will be coordinated with the city of Ukiah Inspector and/or
the contract.tr s representative.
Proposed Fee
We understand that the Project is projected to have a construction pxtriod of 180 working days. Because
construction projects often proceed on a schedule that is unforeseeable, we have estimated our costs
based upon an assumption that our scope of services will be completed in 38 nonconsctiutive working
clays. Our estimated fees art! based on daily rates, including two hour onsite, plus travel expenses, for
the tlunlbLr of working days anticipated for our scope of work on the Project. St IN does not have
minimum time charges per site t isit and eve will invoice the City only for time spent performing the
required services. Where possible, we will combine multiple tasks into one trip it) reduce testing and
inspection costs for the project.
