HomeMy WebLinkAboutSHN Consulting Engineers & Geologists, Inc. 2017-05-25*,tt- dvP 16-1 & - 2�,9 A-3 CITY OF UKIAH AMENDMENT NO. 3 TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES DATED MAY 9, 2016 TERMS AND CONDITIONS All terms and conditions of the Agreement for Professional Consulting Services dated May 9, 2016; the First Amendment dated September 22, 2016; and the Second Amendment dated September 22, 2016 shall remain in force and effect except as modified by this third amendment. AUTHORIZATION Upon execution of this amendment, service provider is authorized to provide the service described under the Scope of Additional Work identified in this Amendment No. 3. SCOPE OF ADDITIONAL WORK Work authorized by this amendment shall include that work described in the November 8, 2016 letter proposal from SHN Consulting Engineers & Geologists, Inc. A copy of said letter is included in this third amendment. COMPENSATION Compensation for the performance of the additional work shall be made on a time and expense basis at Consultant's customary fees. Main St.- Willits, CA 95490-3977. 707/459-4518 • FAX: 707/459-1884 • Wllitsinfo@shn-engr.com Reference: 416056.100 November 8, 2016 Richard Seanor Deputy Public Works Director 300 Seminary Avenue Ukiah, CA. 95482 Via email: rseanor@cityofukiah.com Subject: Engineering Services Contract Amendment - North State Street Intersection, Traffic Signalization, and Storm Drainage Improvements - Low Gap Road/Brush Street Dear Mr. Seanor: Under your direction, we are submitting a final estimate to amend to our existing engineering services agreement for the preparation of construction plans, contract documents, and to provide assistance during bidding for the subject North State Street Intersection Improvements. After completing a set of plans and contract documents which addressed the previously discussed additions to the project plans we have performed additional revisions to the plans, specifications, and contract documents to include the following items: • Add utility services and details for the southeast comer parcel to the plan sheets per new documents subsequently provided by Mr. Seanor after being found in old project files. • Add new chain link fencing and gate along the length of the property on the south side Brush Street. • Modify the drainage piping from the Ceja Tire parking lot drain inlet. • Perform engineering analysis to look at street grades and existing utility grades to see if a temporary storm drain from the northwest comer of the intersection could be constructed to an existing drain inlet at another comer. • Performing additional modifications to the contract documents to be consistent with new City of Ukiah formats after printing and binding of a final set of contract documents. • Perform revisions to the Notice to Bidders document and add a bidder's checklist, adding City of Ukiah sample insurance forms, together with renumbering of the table of contents after each modification or addition to the contract documents. • Creating a specification and bid item for the Contractor to perform Public Noticing of traffic control and closures in the intersection. • Create a specification for Night Construction and a bid alternate item. • Perform research for the estimate cost of Night Construction and amend the Engineer's Opinion Cost for the project. • Prepare a new set of pdf plots of the plans and a pdf file of the contract documents after performing revisions adding the various notes and details referred to herein. • Print a new set of original full size plans with the final revisions. • Print and bind three new sets of contract documents for submittal to the City of Ukiah. To date our total approved budget for the North State Street Intersection improvements is $67,426. Our total billing invoiced to date is $68,031.45 as of September. In addition we would like to reserve a budget of 8 hours for bidding assistance if so needed during the next few weeks of the bidding process. SHN would respectfully like to request an amendment to this contract in the amount of $13,269 to complete our engineering services through bid the opening of this project on November 30, 2016. The total budget for Engineering design services for the North State Street Intersection, Traffic Signalization, and Storm Drainage Improvements — Low Gap Road/Brush Street would be $80,695.
