HomeMy WebLinkAboutAnn Baker Landscape Architecture 2015-05-21; Amendment 1 2015-12-15; Amendment 2 2017-02-24COU No. 1415200-A2 CITY OF UKIAH AMENDMENT NO.2 TO PROFESSIONAL SERVICES CONTRACT 1415200 BETWEEN ANN BAKER LANDSCAPE ARCHITECTURE and THE CITY OF UKIAH This Amendment No. 2, entered on February 24, 2017, revises the Agreement for Professional services dated May 21, 2015 between the City of Ukiah and Ann Baker Landscape Architecture for professional consulting services relating to the construction management/administration and exhibit development services for the Grace Hudson Museum Nature Education Facility Project. This Amendment No. 2 adds to the scope of work Task 4: Museum CCHE Grant Construction Documents & Construction Administration and Observation (T&M), as per Exhibits A and B. Additional cost for this work is S33,695.00. As a result, the revised total contract amount is not -to -exceed S371,500.00. Except as expressly amended by this Amendment, all other terms remain unchanged and in full force and effect. A XJC'& - w & k0 f DATE: KRISTINE LAWLER, CI Y CLERK EXHIBIT "A" Ann Baker Landscape Architecture Grace Hudson Museum Nature Education Project Scope of Work for Amendment #2 to the Construction Administration and Observation Contract PO#1415200 The following scope of work describes the tasks and deliverables the Consultant Team ("Consultants") will follow to provide additional services to create construction docu- ments for additional project elements, to coordinate and lead construction observation services to implement the CCHE Museum Security Grant and complete remaining project elements. TASK 4: MUSEUM CCHE GRANT CONSTRUCTION DOCUMENTS & CONSTRUC- TION ADMINISTRATION AND OBSERVATION (T&M) Subtask 4.01 Construction Document Support for CCHE Museum Security Grant Items Consultants will coordinate and develop construction and bid documents for fencing and gates, and security camera elements. Deliverables: Fencing, Gate and Security element bid documents, Meeting Agendas, Minutes and Facilitation. Subtask 4.02 Construction Administration & Observation for CCHE Museum Security Grant Items Consultants will provide construction administration and observation for the deliver- ables of the CCHE Museum Security Grant. Specifically these items include: functional and decorative pedestrian gates, cable trellis fencing, historic split rail fencing, security camera conduit and CAT 6 cable, coordination of camera installations. ABLA Amendment #2 January 31, 2017 Deliverables: Construction documents for Garden Structures: Plans, Technical Specifi- cations, Special Provisions, Engineer's Estimate and Bid Sheet Subtask 4.03 Extended SWPPP and Surveying Services This subtask is for additional Civil services related to the extended construction period required by the phasing and the continuation of Project 4 into a very wet rainy season. Consultants will continue monitoring and complete close out procedures for the project SWPPP, and provide additional survey and grade checking and drainage evaluation services as directed by staff. Deliverables: SWPPP Monitoring, Testing and Documentation, Survey Move Ins, Civil SKs and Mtg. Minute Notations Subtask 4.04 Protect Team Extended CA/CO Services Due to project phasing and the extended construction period, additional project team services is needed to close out Construction Administration and Observation. In addi- tion as typically occurs in a site renovation, unforeseen issues needing resolution arose during the course of construction, requiring additional staff time and site visits to re- solve. Deliverables: Meeting Agendas and Minutes, Supplemental Instructions and SKs, Project Coordination documents Limitations and Exclusions on Scope of Services Construction Phase Limitations: The objective of construction observation is for Consultants to see if the Work is pro- gressing in general conformance with the contract documents and according to the design concept. Site observation allows the design consultant to answer questions or clarify and interpret documents for the Contractor in a timely way. As outlined above, Consultants will visit the site at intervals appropriate to the stage of construction, in or- der to observe the progress and quality of the work completed by the Contractor. Such visits and observations are not intended to be an exhaustive check or a detailed inspection of the Contractor's work but rather are to allow Consultants to become gen- erally familiar with the Work in progress and to determine, in general, if the Work is pro- ceeding according to the Contract Documents. Based on this general observation, Consultants shall keep the Client informed about the progress of the Work and shall advise the Client about observed deficiencies in the Work. Consultants shall not supervise, direct or have control over the Contractor's work nor have any responsibility for the means, methods, techniques, sequences or procedures selected by the Contractor, nor for the Contractor's safety precautions or programs in connection with the Work. These rights and responsibilities are solely those of the Contractor in accordance with the Contract Documents. Consultants shall not be re- sponsible for any acts or omissions of the Contractor, any subcontractor, or any entity ABLA Amendment #2 January 31, 2017 performing any portions of the Work or any agents or employees of any of them. Con- sultants do not guarantee the performance of the Contractor and shall not be respon- sible for the Contractor's failure to perform it's Work in accordance with the project schedule or in accordance with the Contract Documents or any applicable laws, codes, rules or regulations. Specific Exclusions from this Scope of Work: Construction Management Testing and Inspection Note: this proposal assumes all previous agreements, limitations and descriptions de- scribed in the original scope of work and the project contract will apply herein.
