AUTHORIZATION Upon execution of this amendment, service provider is authorized to provide the service described under the Scope of Additional Work identified in this Amendment No. 2. SCOPE OF ADDITIONAL WORK Work authorized by this amendment shall include that work described in the October 28, 2016 Amendment for Engineering Design, Geotechnical and Construction Oversight Services for Water Well #4 Rehabilitation and New Water Well #9 Project. A copy of said letter is included in this first amendment at Attachment A. COMPENSATION Compensation for the performance of the additional work shall be in accordance with Attachment A./. -Expenses shall not exceed a maximum total cost of $102,938 GHD, INC. (GHD) is pleased to provide the City of Ukiah (City) with this proposal for additional bid period and construction period services for the Water Well #4 Rehabilitation and New Water Well #9 Project, dated April 16, 2014 (Contract No. 1314-200). This amendment request is for additional activities associated with the following: • Specification 16-01: o Rebidding the construction installation of Replacement Well #4 and New Well #9. o Attending the pre-bid meetings. o Additional construction phase activities associated with the Contractor's submittals required prior to well construction o Additional field activities during the drilling, installation, development and testing of Test Well #4, Production Well #4, Test Well #9 and Production Well #9. o Creation of graphic representation of Well #9 relative to Orr Creek for public presentation to City Council. • Specification 16-02: o Attending the pre-bid meeting. o Additional design and construction services to increase the pumping capacity at Well #4 o Additional design and construction services to decrease the pumping capacity at Well #9. Specification 16-01: Additional Bidding and Construction Support Services The scope of services for bid support services is similar in both Contract No. 1314-200 for design of Well #9, and Amendment No. 1 which added design of Well #4. Bid support services are provided as subtasks under Task 10 and Task 22. The scope was limited to responding to questions from bidding contractors 84/11192/10/GHD Amendment 2 Welll 4 & 9 Scope-R2.docx and as -needed technical support for the City. The bid support scope did not include GHD attendance at pre-bid meetings. At the City's request GHD attended the first pre-bid meeting for Specification 16-01. Bids were received from two drilling companies. Both bids had errors in response that caused the City to reject both bids. One bidder took exception to the requirements of the specification and the other did not acknowledge receipt of all addendums. GHD clarified the requirements in the specifications and drawings, and reissued a new set of Mylar drawings for bidding. GHD also attended the pre-bid meeting for the rebid of Specification 16-01. The scope of work associated with Contractor submittals required prior to well construction includes reviews of multiple Contractor submittals and resubmittals of the project construction schedule, well abandonment plan, water pollution control plan, and premobilization submittals. Contractor submittals were significantly lacking in information required by the contract. GHD's project team also attended multiple unbudgeted conference calls and meetings with the City and Contractor to assist in submittal requirements and to assist the Contractor in preparing the required plans and submittals. The construction contract schedule was extended beyond the contract working days. As requested by the City, GHD provided additional field observation and construction documentation support services throughout the schedule extension. The Contractor struggled to develop Well #4, requiring multiple remobilizations to the site to perform additional well development and CCTV video inspections. GHD provided additional field observation services and technical advice during this time including reviews of two additional CCTV inspection videos of the well. Worked with the City and NorCal Pump and Drilling to develop a Mud Plan prior to the drilling of new Well 9, to minimize the impact to the formation from the drilling fluid. GHD provided oversight during the entire drilling process, well installation and development of the new Well 9. GHD provided additional support to the City during the setup and performance of the well pump test and discharge water management. Water management required additional support due to the increased flow achieved from expected rate. At the request of the City, GHD prepared a graphic of Well #9 relative to Orrs Creek including presentation of a -log and lithologic data for public presentation during a City Council meeting. The graphic will be conceptual in nature but based on actual field data, and will show and label well features, casing, perforations, clay / gravel / sand layers, static ground water, groundwater draw down during sustained pumping, and Orrs Creek water level. The intent of the graphic is to demonstrate to the City Council that there is no hydrologic/ hydraulic connection between the groundwater supplied by Production Well #9 and surface waters in Orrs Creek. Specification 16-02 Site Design Revisions The scope of services was limited to responding to questions from bidding contractors and as -needed technical support for the City. The bid support scope did not include GHD attendance at pre-bid meetings. At the City's request GHD attended the pre-bid meeting for Specification 16-02. The scope of work associated with well pumping capacity is related to the estimated production of Well #4 following completion of well construction and development being greater than the design production, and the estimated production of Well #9 following completion of well construction and development being less than the design production. Both Well #4 and Well #9, and their related building and site piping and equipment were designed based on a firm,pumping capacity of 900 gallons per minute (gpm) utilizing 100 horsepower pumps. Upon completion of the monitoring/test wells at each site and pump testing of those wells it was determined Well #4 is capable of achieving a firm pumping capacity of 1,300 gpm with acceptable drawdown, and Well #9 is capable of achieving a firm pumping capacity of 700 gpm with an acceptable drawdown. The City has approved construction of both wells and site improvements. Revisions to the construction documents are required to increase the pumping capacity at Well #4 and decrease the pumping capacity at Well #9. Also Well #9 was installed at a different location than was shown on the drawings and staked in the field. These revisions include: • PCO #001: Revisions to the specified pump duty condition and pump and motor horsepower at Well #4; • PCO #001: Revisions to Well #4 electrical service size, transfer switch, conduits and conductors, and associated electrical equipment to support a higher horsepower pump; • PCO #001: Revisions to electrical equipment at Well #4 to support a higher pumping rate; • PCO #002: Adding a Mendocino County Standard commercial driveway entrance and wider gate at the Well #9 site; • PCO #002: Revising the location of the Well #9 well head, enclosure and associated electrical and piping to reflect the location the well was actually constructed. • PCO #003: Revisions to the Well #4 site utility plan and development of a PCO to remove items of work which were completed by others. • PCO #004: The addition of solenoids on valves and related electrical and instrumentation improvements in the well buildings at Well #4 and Well #9 to allow for automated or remote operations. • Preparing, issuing, reviewing and negotiating change orders associated with the design revisions. These design revisions require changes to construction contract drawings and the specifications. The table below summarizes the additional professional service fees for these services. The table below is a summary of the overall project budget reflecting approved and pending amendments.
