AMENDMENT NO. 2
TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES
DATED MAY 9, 2016
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Consulting Services dated
May 9, 2016 and the First Amendment dated September 19, 2016 shall remain in force and
effect except as modified by this second amendment.
AUTHORIZATION
Upon execution of this amendment, service provider is authorized to provide the service
described under the Scope of Additional Work identified in this Amendment No. 2.
SCOPE OF ADDITIONAL WORK
Work authorized by this amendment shall include that work described in the
August 30, 2016 letter proposal from SHN Consulting Engineers & Geologists, Inc. A copy
of said letter is included in this second amendment.
COMPENSATION
Compensation for the performance of the additional work shall be made on a time and
expense basis at Consultant's customary fees. Expenses shall not exceed a maximum
total cost of $25,266.

Subject: Engineering Services Contract Amendment - North State Street
Intersection and Storm Drainage Improvements - Low Gap Road/Brush
Street
Dear Mr. Seanor:
Under your direction and discussion with Tim Eriksen, Public Works Director, we are submitting
an estimate to amend to our existing engineering services agreement for the preparation of
construction plans, contract documents, and to provide assistance during bidding for the subject
North State Street Intersection Improvements. We have been directed to 1) add an ADA accessible
ramp on the existing southwest curb return, 2) design a complete ADA ramp with a new 35 foot
radius curb return for the southeast corner, and 3) design frontage improvements along the south
side of Brush Street from North State Street to the end of the existing sidewalk improvements
across from the intersection with Mazzoni Street and within the right of way previously purchased
by the City of Ukiah.
These new frontage improvements include retaining walls, driveways, new utility stubs for future
development of the adjacent parcel, and special foundation support for the existing structure which
will be effected by the widening and construction of the Brush Street improvements. The widening
of Brush Street will allow for a left turn lane with reasonable stacking distance and safer vehicle and
pedestrian flow on Brush Street. These improvements will also facilitate a full left turn traffic
signalization of the intersection. A new traffic signal pole with an overhead mast will be added to
the southeast corner of the intersection. A complete left turn arrow signal plan is being prepared
for the intersection. In addition, we are redesigning the storm drains for the intersection to include
all four corners of the intersection and to accomplish the full abandonment of the existing steel
culverts which have been found to be failing. Due to the number of existing underground utilities
crossing this intersection, the design of the storm drain is complicated and will be very deep to pass
under the existing utility piping.
We are setting up the plans and contract documents to bid this work as one project but will phase
the construction work. Phase One would be performed as soon as the project can be bid and
awarded this fall because of the importance of rebuilding the storm drain that was severed to
construct the new sewer main in North State Street. During the sewer main construction, it was
noted that the existing steel storm drains crossing State Street were all perforated with rust holes.
During the construction of the new storm drain, it is anticipated that new curb returns would be
constructed along with the relocations of existing traffic poles and installation of new poles and
traffic signal modules to create left turn arrows in all directions. It is important that the storm
drainage work be performed as a first priority along with the other associated improvements,

unless a temporary alternative can be installed. We would then suggest that after the storm drains,
curb returns, the shoulder construction adjacent to the new curb returns, and the signalization work
and Phase One street striping is completed that work be suspended until spring.
In the spring, Phase Two construction would resume to address the improvements along the Brush
Street frontage, reconstruction of the structural section of Brush Street together with pavement
rehabilitation in State Street and Low Gap Road and in the intersection. Once the new pavement
rehabilitation and reconstruction is completed, the Phase Two striping plan would be applied for a
finished project.
We anticipate having 95% of plans and contract documents ready for the City of Ukiah staff to
review during the week of September 6, 2016. We assume that some minor revisions may be
necessary after review by staff and we would then prepare a final set of plan and contract
documents as quickly as possible, signed by a California Registered Civil Engineer and submitted
to the City of Ukiah. We will provide the City ten (10) full-size sets of plans together with PDF files
of the same. We will also provide three (3) sets of bound Contract Documents, a Word document
file and a PDF file of the Contract Documents. We will prepare an Engineer's Opinion of Cost for
the proposed improvements. Providing the City with assistance during bidding to help answer
contractor questions as needed and preparing a bid tabulation of the submitted bids will also be
performed. Our estimated amendment to the engineering services agreement which includes
providing the aforementioned services, final signed plans for bidding, and bidding assistance is
$25,266.
