Ukiah, CA 95482 You are hereby directed to make the herein described changes from the contract plans and specifications or do the following described work not included in the contract plans and specifications: Change requested by: City Engineer. Description of Changes - Itemized Breakdown ESTIMATED DECREASE To Contract Amount ESTIMATED INCREASE To Contract Amount 1. Add thermoplastic pavement markings: "SLOW SCHOOL XING" in advance of crosswalk, both directions and in each lane (total 4 applications), in accordance with California MUTCD standards, at the agreed lump sum price of $8,970. S8 970 2. Remove item from CCOa 1 "Remove existing thermoplastic crosswalk striping," at the agreed lump sum price of$1,960.00. $1,960.00 ESTIMATED NET CHANGE IN CONTRACT PRICE S7+010.00 Justification (Detailed and Concise): Item 1: The pavement markings "SLOW SCHOOL X1NG" are required for uncontrolled crosswalks in school zones, though was omitted from the original project plans and bid schedule. Item 2: "Remove existing thermoplastic crosswalk striping," was listed as an extra work item in CCO 41, however it should be removed from extra work since it is listed in the bid schedule and a lump sum cost was provided by the Contractor. The amount of the Contract will be increased by the estimated sum of seven thousand ten dollars and no cents ($7,010.00). The contract total including this Change Order will be: one hundred seventy-nine thousand four hundred sixty-nine dollars and sixty-three cents ($179,479.63). The contract period provided for completion will be increased due to the need to re -order 2 signs due to incorrect size, and obtaining needed parts for the flashing crosswalk system, which was beyond the control of the Contractor. The new completion date will be June 30, 2016. This document will become a supplement to the contract and all provisions will apply hereto.
