AGREEMENT FOR PERFORMANCE OF
MUSICAL ENTERTAINMENT
This Agreement made on this day, Saturday April 2 2016 by and between CITY
OF UKIAH (hereinafter referred to as CLIENT), and COLEMAN BURKE dba PRIDE 8s JOY
(hereinafter referred to as P 8c J). The purpose of this Agreement is to state the terms,
conditions and services P 85 J will render to CLIENT.
PERFORMANCE:
DATES: SUNDAY JUNE 12 2016
LOCATION: SUNDAYS IN THE PARK - TODD GROVE PARK
SHOWS: DANCE SETS
APPROX. 6:00 PM - 8:30 PM
SET UP: APPROX. 2:00 PM
ATTIRE: OUTDOOR
PAYMENT:
In consideration of said Performance,CLIENT agrees to pay,$5000.00
(FIVE THOUSAND AND NO/ 100 DOLLARS )
Deposit in the amount of $2500 payable to PRIDE & JOY to be received
by April 18 2016
THANK YOU
Balance due on evening of engagement, prior to Performance. Said balance
to be paid in CHECK payable to PRIDE & JOY.
CANCELLATION:
CLIENT may cancel any performance prevented by fire, casualty, strike, or any other cause not
within the control of CLIENT, by written notice no later than 365 days before scheduled
engagement date. Failure of CLIENT to comply with this cancellation provision will result in a
cancellation fee of $ 1,000.00 and loss of deposit.
P & J may cancel any performance prevented by illness, accident, riots, strikes, epidemic or
any other condition beyond the control of P & J. If P & J has received a deposit for said
performance, P & J will return said deposit within ten days of cancellation.
SOUND, STAGE, AND LIGHTING REQUIREMENTS:
A. Sound system for event to be provided by : CLIENT
B. Lighting for event to be provided by : CLIENT
In the event P & J will be providing sound and lights, CLIENT agrees to provide
six (6) twenty (20) amp circuits for said performance.
C. CLIENT agrees to provide a stage area necessary for performance.
12' X 24' minimum If this requirement is not feasible,
all alternatives must be discussed with COLEMAN BURKE.
All precautions must be taken to insure that said stage is securely
assembled, (ie. no cracks or spaces between sections) to guarantee
safety of all band members during performance.
DRESSING ROOM:
CLIENT agrees to make the best possible effort to provide a private dressing area
equipped with mirrors and adequate seating for 13 band members and crew.
CLIENT agrees,when possible, to provide ten (10) clean hand towels for
P&J's use prior to performance.
FOOD AND BEVERAGE:
CLIENT agrees to provide
P & J with thirteen (13) hot balanced protein based meals ( chicken, vegetable starch)
CLIENT agrees to provide the following BEVERAGES:
One case NON CARBONATED WATER
Six assorted soft drinks (Coke or Pepsi and 7 UP )
Three Beers
Three Red Bulls
One quart Cranberry Juice
One quart Orange Juice
One pot hot coffee
SPECIAL PROVISIONS
THE PARTIES ACKNOWLEDGE THAT EACH HAS READ THIS AGREEMENT,
UNDERSTANDS IT, AND AGREES TO BE BOUND BY ITS TERMS.
