This Amendment No. 1, entered on August 24, 2015, revises the Agreement for Professional
services dated May 13, 2015 between the City of Ukiah and Crawford & Associates ,
Inc. for professional consulting services relating to the Engineering Services for the
Hydroelectric Plant Grating and Handrails.
This Amendment No. 1 adds Exhibit A -1- Project: 15- 223.1 -City of Ukiah Hydroelectric
Plant Gratings/Handrails. Additional cost for this work is not -to- exceed $4,815.00.
As a result, the revised total contract amount is not -to- exceed $18,653.00
Except as expressly amended by this Amendment, all other terms remain unchanged and in
full force and effect.

SUBJECT: PROPOSED AMENDMENT 1 TO CONTRACT #1415194
FOR ADDITIONAL PROFESSIONAL CONSULTING SERVICES
Dear Mary,
Based on discussions in a meeting with Director Mel Grandi, Steven Beaman, and Jim O'Brien on August 18,
2015 at the City offices, it was determined that additional grating and handrails are needed in an area not
included in the original RFP. This is the area south of Unit 1 TSV and east of Unit 1 Generator. Grating will be
supported one to two -steps above the floor level in order to cross conflicting pipes, conduits, supports,
grounding cable, equipment pad, and equipment base plate, and to address the differing floor levels adjacent
in this area.
Scope of Work:
This work will require additional effort to visit the site to measure the areas and document the conditions; to
prepare an additional 8 -1/2 "x11" layout sheet for the area; to prepare several additional details as the
solution to the requirements of this area differ from details of construction to be used in other areas; and to
expand the bid quantity sheet and the Opinion of Probable Construction Costs; and to address questions
during bidding. I have included an incremental increase for the structural engineer who is performing the QA
and preparing the calculations for the building permit. No additional specifications will be required for the
additional area as the work is of the same nature as the original scope. Enclosed as Exhibit A — Task Hour
Breakdown is a detailed spreadsheet outlining the efforts and the additional costs. All other Terms and
Conditions of the Agreement between the City of Ukiah and Crawford & Associates, Inc. will remain the
same.
Schedule and Cost:
The estimated cost to provide these additional services is $4,815, which would increase the overall contract
amount to $18,653. With the outstanding questions, and logistics surrounding the ability to meet, we have
just passed the originally anticipated completion date of early August. Based on the current status of the
design, I anticipate that approximately 4 weeks will be necessary to complete the final documents and get
them to the structural engineer for his work, and expect that he can turn them around in one week. We will
do our best to better that time frame.
Thank you for considering this proposed Amendment. In the meantime, I will also await the results of the
consideration of whether to relocate the heat exchanger equipment or to modify the backboard support for
the gauges west of the heat exchanger to free up enough space to provide the required walkway width.
