AMENDMENT NO. 1 TO THE AGREEMENT FOR PROFESSIONAL SERVICES
DATED AUGUST 14, 2007
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Services between the City of Ukiah (the
City) and Sangiacomo Landscape (the Consultant), dated August 14, 2007 (the Agreement) shall
remain in full force and effect except as modified by this Amendment No. 1.
AMENDMENT AUTHORIZATION
The City may amend the Scope of Work of the Agreement pursuant to the provisions of paragraphs
2.2 and 4.2 of said Agreement. The Consultant shall perform any added work and/or not perform any
deleted work upon the execution of this Amendment. The Added work shall be paid for on an Agreed
Lump Sum basis.
AMENDED AGREEMENT PROVISIONS
Paragraph 2.0, "Scope of Services", of the Agreement is modified by the addition of the
following paragraph:
2.1.1 Added Work: The Consultant shall show on the Project Drawings the planting of 36 new trees
instead of 21 trees as called for in Attachment "A". The Consultant has determined that there are a
total of 12 trees that need to be removed to accommodate the proposed bridge: 8 trees under the
proposed bridge footprint and 4 near enough to the bridge requiring removal. Based on a required 3 to
1 tree replacement program, 36 new trees will be need to be planted to replace the 12 trees to be
removed.
Paragraph 4.1, "Basis for Compensation", of the Agreement is modified by the addition of the
following:
The Consultant will be paid an additional $545.00 for performing the added work as described in this
Amendment. The Total Lump Sum compensation including the cost for the added work is changed to
$3,790.00.
EFFECTIVE DATE OF THE AMENDMENT
The effective date of the Amendment is the date the Amendment is executed by the City.
AMENDMENT EXECUTION
The City and the Consultant agree to the provisions of this Amendment by signing below:
