The charges for this project are being made under the following installation provisions: Electric- Standard Installation Under Option 1 . The total payment due , as shown in the Extension and Service Agreement , is $21 , 812 . 71 . Additional detail for these charges is shown in the Exhibit A of the agreement. You must specify your payment option selection on the second page of the Extension and Service Agreement (Refundable/Discount Option Selection) even though you have no initial refundable payment due under Rule 15 . Your payment option selection will determine how any adjustments will be made for non-compliance with allowances that are granted . Effective January 1 , 1992 , the State of California has considered Contributions In Aid of Construction (CIAC) as taxable in all situations where Federal tax on CIAC's apply. Some of the charges shown above include an Income Tax Component of Contribution (ITCC) at the current rate of 34% for electric facilities . ITCC will be either refundable or non- refundable in accordance with the corresponding contribution . The prices quoted for this job are valid for ninety (90) days. If construction has not started within this ninety day period , a revision of these charges may occur. PG&E construction will usually start approximately three weeks after receipt of the signed agreements, payment of all charges specified above , and your readiness to receive service . The construction requirements for this project have been previously provided to you and all facilities must be installed as shown on the PG&E construction sketch and standards provided . In addition , your work must comply with certain requirements which are contained in PG &E's Electric and Gas Service Requirements manual (Greenbook) . Any deviations must be approved by the PG&E Inspector, with any change-orders signed before work can proceed . We are willing to proceed with due diligence to accomplish this preliminary work after you provide us with a project deposit of $2,000. 00 . This deposit is based on current costs and will be firm for a period of ninety days after the date of this letter. If this letter agreement is not executed and the payment made by you prior to the expiration of the ninety day period, it may be necessary for us to re-estimate the amount of the payment and prepare another letter agreement. In addition, please complete and return the enclosed Application for Service and the Installation Option Form with your deposit, along with your proposed service route and meter location (sketch preferred) . You may, at any time, request that we stop the preliminary work on this project with the understanding that the actual costs that have been incurred by us to that time will be deducted from your deposit, and the excess, if any, will be refunded without interest. The project deposit includes costs to be incurred by PG&E for labor, materials, and supplies, transportation, tools, payroll taxes, insurance, supervision and other allocable direct costs, in accordance with our standard accounting practices . This deposit shall be refunded to you or otherwise credited against amounts due when we have completed the installation of our electric mainline extension facilities in your project and under the terms and conditions of the final agreement. All proposals made in this letter are based on tariffs (rates and rules) in effect at this time. Changes to those tariffs ordered by the California Public Utilities Commission (CPUC) may require changes in our proposed work. Those changes may also require modification of our proposal and charges. Rick Kennedy June 3 , 1996 Page 2 It is the policy of PG&E to inform the applicant of our "change-order" policy. PG&E will contact the applicant at specific critical points of the job process to verify the information provided (i. e. , load information and plan version) . After notification, if the applicant requests or requires any change to the agreed upon plans, then this change will be billed to the applicant using PG&E's change-order form. An authorized agent's signature will be required before any work will be started. If PG&E is the trenching agent, trench costs for the telephone and cable television companies shall be collected from the applicant in advance of construction. It is the applicant's responsibility to contact the local telephone and cable television companies to determine if trench reimbursements are due. COMPETITIVE BIDDING, When Applicant selects competitive bidding, the extension may be installed by Applicant's qualified contractor or sub-contractor in accordance with Utility's design and specifications. Under this option, the following applies: a. Once Applicant chooses competitive bidding, the choice is final; b. PG&E's total estimated installed project-specific cost will apply regardless of whom Applicant selects to perform the installation; c. . Upon acceptance by PG&E ownership of such facilities will transfer to PG&E; d. Applicant shall pay to PG&E, subject to refund, any PG&E project-specific estimated costs associated with the extension, including estimated costs of design, administration, and installation of any additional facilities and labor needed to complete the extension; , e. PG&E's total project-specific estimated installed cost is subject to the refund and allowance provisions of this rule; f. Applicant shall pay to PG&E, as a non-refundable amount, the cost of inspection; and, g. Only duly authorized employees of PG&E are allowed to connect to, disconnect from, or perform any work upon PG&E's facilities. Electric workers shall be properly qualified (Qualified Electric Worker, Qualified Person, and etc.) as defined in State of California High Voltage Safety Orders (Title S, Chapter 4, Subchapter 5, Group 2); and c. comply with applicable laws (Equal Opportunity Regulations, OSHA, EPA, and etc.). Further, an applicant for service who intends to employ a QC/S should consider whether the QC/S: a. is technically competent (including, as appropriate, work experience on facilities up to 50,000 volts, plastic fusion, welding, and etc.); be has access to proper equipment; ce demonstrates financial responsibility commensurate with the scope of the contract; do has adequate insurance coverage (Workers' Compensation and Employer's Liability, Commercial General Liability, and Business Auto); and e. is able to fumish surety bond for performance of contract, if required.
