AMENDMENT2
TO
PROFESSIONAL SERVICES AGREEMENT
BETWEEN
LAMINAR ENERGY SERVICES and THE CITY OF UKIAH �
This Amendment No. 2, entered on November 9, 2012, revises the Agreement for
Professional services dated DecemUer 19, 2011 beriveen the City oF Ukiah and Laminar
Energy Services for professional consulting services relating to Phase 2 Engineering
Services at the Lake Mendocino Hydro Faci]ity for the repair of the Penstock.
tlus Amendment 1Vo. 2 adds addifional scope of work, as well as an additional not-to-
exceed cost of $21,400, as identified in the proposal letters from Laminaz Energy �
Services dated November 7, 2012, and mazked as Attachments"A"and"B". �
As a result,the revised total agreement amount is not-to-exceed$77,600.00.
Except as expressly amended by this Amendment, and Amendment 1, all other terms
remain unchanged and in full force and effect.
