execed cost of $26,200.00, as identified in tl�e proposal lettcr fiom Laminar Giergy
� Scrvices dated Ja�uary 14, 2012, and marked as Attachment "A".
� As a result the ravised total agreement amount is not-to-exceed $56,200.000

Su6ject Second Proposed Addendum to]ob #L1112A-Ukiah• Penstock Phase 2.
Dear Mr. Grandi:
[t is proposed that Laminar Energy Services,Inc. provide Ownei's Engineering consultation I
for the additional scope items listed below. These items are listed as item numbers 9
through 12 as additions to the existing job number L1112A-Ukiah,[tems 1 through 5 I
previously approved. Currently,there is a separate addendum Scope-of-Work Changes
[L1112A(al),[tems 6 through 9] pending approval.
The total estimated labor time for this proposal is Eighteen (18) man-days based on
realistic worse case conditions for items 9 through 12, below. Examples of these
reasonable worse case conditions would be problems encountered during valve removal
and reinstallation, separate days to witness each Factory Witness Test,and conducting
startups on the two units at separate site visits.
The scope of work proposed is as follows:

9) Mechanical Contractor Bid Specification work:
a. Write and deliver a bid specification package for the mechanical
contractor work scope to correct the Unit 1 and Unit 2 Turbine Shutoff
Valve (TS� problems.
b. Develop and provide a recommended bidder's list.
c. Conduct a Bidder's Mandatory Site Bid Walk-down.

10)Oversee the mechanical contractor site work and provide quality assurance
inspection services during the removal and reinstallation of both TSV's.
11)Review,approve,observe,and obtain final documentation of the TSV Factory
Witness Leak and Stroke Test conducted by the valve repair shop.
a. [ncludes verification that the overhauled TSV's meet bubble tight leak
standards at rated pressure through a formal hydrostatic test.
b. Opening/closing forces are within the standard set by ANS[/AWWA
C504 standards set for butterfly valves.
12)Provide startup and commissioningservices during the plant restart. I
SCHEDULE
The work will start as soon as a Purchase Order or other authorization is received
authorizing this Work Scope. Site access will be coordinated with customer due to the
nature of the work location and associated restrictions.

PRICING AND TERMS OF PAYMENT �
1. The above discussed scope of work plus expenses is estimated at a total price
of$26,200 (twenty six-thousand two-hundred US do]Iars).The work will be
performed on a time-and-materials"not-to-exceed"basis at a straight- time
billing rate as defined in the existing contract of$180 per hour (plus i
any expenses). Any required materials are cost plus 10%.

2. Paymentterms are Net 30 days from date of receipt of correct invoice. i
Laminar Energy is responsi6le to assure all documentation is correct and
complete when submitted. Late fees of 1-1/2%per month may be assessed
for payment received 30 days after invoicing. ,
3. This offer is extended for 60 days after the date of this proposal. I
4. Laminar Energy Services, Inc.warrants and guarantees that it will perform
its services in accordance with the standards of care,thoroughness,
competence normally practiced by recognized firms in the industry in
performing services of a similar nature at the time of performance of the
services.
