TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES '
DATED APRIL 6, 2012
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Consulting Services dated
April 6, 2012 shall remain in force and effect except as modified by this first amendment.
AUTHORIZATION
Upon execution of this amendment, service provider is authorized to provide the service
described under the Scope of Additional Work identified in this Amendment No. 1 .
SCOPE OF ADDITIONAL WORK
I
Work authorized by this amendment shall include that work described in the
July 2, 2012 letter proposal from Green Valley Consulting Engineers. A copy of said letter
is included in this first amendment.
COMPENSATION
Compensation for the performance of the additional work shall be made on a time and
expense basis at ConsultanYs customary fees. Expenses shall not exceed a maximum ,
total cost of $1 ,944. !

Re: Contract Amendment#1 for Perkins and Orchard Streets Intersection Improvement Construction
Management and Inspection
Dear Rick:
We would like to request an amendment to our Construction Management (CM) and
Inspection contract for the Perkins and Orchard Streets Intersection Improvement Project, which
is needed to address the increase in our services, especially the CM services. I am hopeful that
Gene has already kept you informed of much of the following services which led to our original
coniract being expended.
The increase in both CM and inspection time has been associated with a number of issues on
the project, a few of which are briefly summarized below:
: Increase in the number of working days which we esTimated on Contract from 40 to 52,
resulting in corresponding inspection and construction management labor costs
� We estimated 10 hours of time to be devoted to 5 RFI's and there were 15 RR's
� We estimated CM time at 2 hours per day and we needed approximately 3 hours a day
to address the various items
� We estimated time to address 6 change orders and we had 1 1 change orders
� The change order management time was estimated at 6 hours and it took 27.5 hours (1
hour for field directive and 1.5 hour for change order for each)
: Additional inspection time associated with the following change orders: CCO 3-cap
existing water main (6 hours), CCO 4- remove abandoned pipe (4 hoursJ, CCO 7 -
Cold-plane AC and overlay �4 hours�, CCO 9 -four electrical changes �8 hoursJ, CCO
10- concrete wall change (4 hours)
: Additional training from the manufacturers rep for ihe wireless vehicle detector system
for submittal review and installation
: Additional coordination with Contractor
Fee 51,944
Attached is a spreadsheet which breaks down our requested fee for your review. It indudes
ihe time needed to complete the project thru ihe punch list and close-out phase. Our
reimbursable budget came under by close to $1500 which helped to offset most of the above,
leaving us only a little short by the $1944. We will bill out the reimbursable on the next invoice
