Dear Mr. Burns:
With the attached letter, the Ukiah City Council has relied upon Ukiah Valley Medical Center Adventist
Health to pay one-half the cost of a construction contract awarded to Ferranti Construction of Redwood
Valley in the amount of $43,110.
This document will serve as an agreement between the City of Ukiah and UVMC for payment of $21,555,
due upon billing by the City of Ukiah. The City's normal procedure is to provide for administrative
approval of change orders not to exceed 10% of the total project cost, without further spending
authority from the City Council. City staff does not anticipate change orders for this project, but should
they occur, UVMC would be billed for one-half of the change order, to a maximum of $2,156. Should
change orders exceed 10%, further expenditure authority would be required from the City Council, and
UVMC could be requested to participate in that cost.
Please sign below and return to me as soon as possible, so that I may sign the construction agreement
with Ferranti in keeping with the direction of the City's Redevelopment Agency Board.
Thank you for entering this partnership with the City of Ukiah and its Redevelopment Agency to provide
improvements for pedestrian safety and access on Hospital Drive.

Dear Ms. Chambers:
Please accept this letter as our agreement to fund one half of the $43,110 for
the cross walk addition connecting, approximately 260 Hospital Drive, with our
OB entrance area. I have personally signed the drawings indicating the correct
location.
It is our intent that the crosswalk in question be located directly in front of the
walkway into the center of the 260 Hospital Drive building.
We agree to fund $21,555 of the total.

I want to personally thank the City for their leadership and planning regarding the approval to
partner with Ukiah Valley Medical Center in regards to installing a crosswalk with curb bulb-out
on Hospital Drive. This crosswalk will be a help to both our employees and patients in crossing
this busy street in a safe manner.
Per your conversations with Terry Burns, President of the Hospital, Ukiah Valley Medical
Center will commit to covering half of the expenses, up to ten thousand dollars ($10,000)
towards the installation of the crosswalk. Please give me a call so we can work out the best
solution for reimbursing the City for the expenses.
We look forward to working with the City to complete this project.
