AMENDMENT NO. 2
TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES
DATED JUNE 24, 2010
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Consulting Services dated
June 24, 2010 and the first amendment dated December 29, 2010 shall remain in force
and effect except as modified by this second amendment.
AUTHORIZATION
Upon execution of this amendment, service provider is authorized to provide the service
described under the Scope of Additional Work identified in this Amendment No. 2.
SCOPE OF ADDITIONAL WORK
Work authorized by this amendment shall include that work described in the
January 3, 2011 letter proposal from Green Valley Consulting Engineers. A copy of said
letter is included in this second amendment.
COMPENSATION
Compensation for the performance of the additional work shall be made on a time and
expense basis at Consultant's customary fees. Expenses shall not exceed a maximum
total cost of $9,983.

Subject: Request for Amendment #2 to our Construction Management & Inspection Contract
Dear Rick:
As we discussed, we are short by $5,730.50 through 12/25/10.
On the amendment #1 request I estimated that part or our time would be spent on the South Dora project.
However the South Dora project shut down for the winter so that didn't happen. Also I estimated that the
Gobbi project would end 12/5/10 however it actually ended 12/13/10.
For 12/26/10 through 1/7/11 1 have estimated that there will be no inspection and the construction
management time will be for "closeout work" as detailed below.
Negotiate claims and change orders with Contractor = 4 hours
Prepare & process 8 change orders = 16 hours
Assemble final pay quantities and prepare final pay estimate = 8 hours
Claims & stop notices were settled so hours = 2
Asbuilt drawings and photos & deliver to the City = 4 hours
Clean up project hard-file & deliver to the City = 3.5 hours
Total hours = 37.5 x $105 = $3,937.50
$3,937.50.+ $5,730.50 = $9,668.00
Asbuilt Drawing mylars = $315.
Total request for amendment #2 is $9,983.00
