AMENDMENT NO. 2
TO AGREEMENT FOR PROFESSIONAL CONSULTING SERVICES
DATED NOVEMBER 2, 2007
TERMS AND CONDITIONS
All terms and conditions of the Agreement for Professional Consulting Services dated
November 2, 2007; and the first amendment dated October 7, 2008; shall remain in force
and effect except as modified by this second amendment.
AUTHORIZATION
Upon execution of this amendment, service provider is authorized to provide the service
described under the Scope of Additional Work identified in this Amendment No. 2.
SCOPE OF ADDITIONAL WORK
Work authorized by this amendment shall include that work described in the
September 1, 2008 letter proposal from Green Valley Consulting Engineers. A copy of said
letter is included in this second amendment.
COMPENSATION
Compensation for the performance of the additional work shall be made on a time and
expense basis at Consultant's customary fees. Expenses shall not exceed a maximum
total cost of $9,550.

Re: Additional Services Associated with Pavement Reconstruction Improvements at Gobbi
Street - Amendment #2
Dear Rick:
Per your request and previous phone conversations, we have enclosed a brief scope of services and
associated fee for the addition of pavement reconstruction to the Gobbi Street Water Main
Improvements and for work associated with the southwest corner of Gobbi and State Streets to
increase the curb return radii and allow for a right turn pocket.
Our Understanding
We understand that the City would like to add an asphalt reconstruction component to the water
main improvement project on Gobbi Street from the limits of Main Street west thru the intersection
of State Street. At this time, the method is proposed to be a "grind and overlay". The work
proposed is outside our existing scope of services and will require additional topographic mapping,
preparation of mapping, coordination with the water main project and design of the pavement
reconstruction, including raising existing utility boxes, etc. to grade.
In addition, the City would like to increase the radius of the SW curb return at Gobbi and State
Streets to facilitate truck traffic making right turns and add a dedicated right turn lane with a
stacking length as feasible with the physical constraints of the corner.
It is also our understanding that the additional work will be advertised with the water main
improvements under the same contract. We have briefly summarized the additional scope of
services and associated fee below.
Topographic Mapping
We will need to extend the original topographic mapping that was obtained to pick up the limits of
the proposed pavement reconstruction work on Gobbi Street. We will pick up surface features and
any underground utilities that are apparent and cross-section the existing road at 50' sections.
Prior to field surveys, we will mark for USA to assist in identifying underground utilities that may be
in conflict. Mapping will include the project area covering a sufficient distance beyond the proposed
improvements to properly design the project.

75% Design and Improvement Plans
The 75% design documents will include the structural section details, horizontal limits proposed for
the curb return, turn lane and the pavement reconstruction improvements, and the vertical profile of
improvements. The plan and profile sheets will indicate existing right-of-way lines and any pertinent
easements, topographic features, and existing utility systems. It is expected that there will two
additional sheets required for the new work, including a plan and profile and striping and
signing/detail sheet. Previous project specifications that were submitted for the water main
improvements will be amended for the additional pavement and concrete work to allow for
subsequent advertising.
An updated cost estimate will also be submitted to allow for budgeting purposes.
100% Design and Improvement Plans
A 100% submittal of the plans will incorporate all comments from the 75% review, as well as results
of utility company reviews of the plans. Final vertical profile, cross-sections, and any miscellaneous
details will be included. Construction documents will be prepared in accordance with the City of
Ukiah Design and Construction Standards. The final cost estimate will updated to incorporate
current bid results.
Fee for Additional Services $9,550

proposed scope of services and associated fee, Green Valley can begin work upon authorization to
